No such thing exists. There are DualDiscs which CAN have DVD-A on one
side and a sandwiched CD on the other but not a hybrid.

True, I forgot about Dual Discs. Technically they aren't Redbook-compatible,
based on their thickness. Some CD players have trouble playing them.

My mistake. I'm familiar with SACD which in many cases are hybrid disks. I
incorrectly assumed that DVD-As could also be hybrid.

Not in the same way ...you have to flip a 'hybrid' DVD-A (a Dual Disc) over to
play the CD side. And as Kal says, the DVD side of a Dual Disc
might be DVD-A, or it might not, e.g., it might be Dolby Digital instead.
